Digital Waiver
By participating, I agree to:
Assume Risk: I realize this is a
physical and mental activity
and accept all risks, including
use of simple tools, lifting small
objects, time pressure, and
being in rooms with others. I
accept risks arising from my
actions and equipment.
Health/Safety: I can
participate safely and not under
the influence of drugs or alcohol.
Conduct: I will follow all rules.
Failure to comply may result in
removal without refund. I agree
to pay a $25 repair fee if I
break or damage props by
mishandling them.
Release of Liability: I waive
and release Stealth Escapes
staff from all liability for injury,
loss or damages. I agree to
hold them harmless from any
claims related to my participation.
Medical Consent: I authorize
Stealth Escapes staff to get me
emergency medical care if
needed.
Photo/Video Permission: I may
be photographed or recorded
and give permission for my
likeness to be used for legitimate
business purposes.
Certifications: I understand this
waiver and sign voluntarily.
Parent/Guardian Consent: I
accept full responsibility for
minors I'm signing for. I consent
to using an electronic signature
and may request a paper copy.
